"abstract": "Rationale:: Full-length ureteral avulsion is one of the most severe complications of ureteroscopy. Although this condition is extremely rare, the absence of standardized treatment protocols poses significant challenges for urologists in clinical management.\n\nPatient concerns:: In case A, the subject is a 48-year-old male, and in case B, the subject is a 63-year-old female. Both patients had full-length ureteral avulsion during ureteroscopic lithotripsy. Case B underwent emergent primary repair involving pyeloureterostomy, ureteroneocystostomy, and omental wrapping; however, the procedure failed to achieve satisfactory functional recovery, resulting in persistent obstruction.\n\nDiagnoses:: Based on clinical history and imaging findings, case A was diagnosed with full-length ureteral avulsion, and case B presented with ureteral stricture following primary in situ repair of full-length ureteral avulsion.\n\nIntervention:: Both patients were transferred to our center for upper urinary tract reconstruction 5 months after the original injury. Following bowel preparation, they underwent totally intracorporeal robot-assisted ileal ureter replacement.\n\nOutcomes:: Both patients demonstrated stable total renal function and adequate drainage from the reconstructed renal units postoperatively.\n\nLessons:: The totally intracorporeal robot-assisted ileal ureter replacement represents a safe and effective treatment in managing full-length avulsion injuries of the ureter. Lifelong follow-up remains essential after reconstructive surgery and should include surveillance for obstruction, infection, metabolic complications, and renal deterioration. Preventive measures need to maintain paramount importance in clinical practice to avert such severe iatrogenic complications.",
