"abstract": "Xanomeline-trospium (Cobenfy) is a novel treatment for schizophrenia that exerts its therapeutic benefit through muscarinic receptors without direct dopamine receptor effect. Xanomeline is a muscarinic M1- and M4-preferring receptor agonist, while trospium acts as a peripherally acting muscarinic antagonist that mitigates cholinergic adverse effects without crossing the blood–brain barrier. Since our initial publication in this journal reviewing data from the EMERGENT clinical trials, additional evidence has emerged that further characterizes the efficacy, safety, and clinical role of xanomeline-trospium in schizophrenia. This review provides an overview of recent evidence. Across pooled analyses of randomized trials, xanomeline-trospium demonstrated clinically meaningful improvements in positive, negative, cognitive, and agitation-related symptoms, with treatment effects observed as early as week 2 on some measures. Benefits were observed in patients with prominent negative symptoms, greater cognitive impairment, and higher levels of agitation at baseline. Long-term open-label studies demonstrated sustained improvement in symptoms and tolerability over 52 weeks among both previously treated and treatment-naïve patients, supporting findings from the 5-week EMERGENT trials. Improvements in functional impairment were also reported. Xanomeline-trospium was well tolerated, with gastrointestinal adverse effects occurring more frequently than placebo, while rates of extrapyramidal symptoms, clinically meaningful weight gain, and metabolic disturbances were low and comparable to or lower than placebo. Indirect comparisons to antipsychotics further suggested favorable benefit to risk profile. These findings provide continued support of xanomeline-trospium as an effective and well-tolerated treatment for schizophrenia. Future research is needed to assess the effectiveness of xanomeline-trospium across different stages of schizophrenia and through head-to-head trials.",
