"abstract": "Background: Burn wound management in children remains challenging, particularly with respect to wound healing, pain, complications, duration of hospitalization, and scar formation. This study aimed to compare nano-silver dressing with platelet-rich plasma (PRP) injection in pediatric burns management. Patients and Methods: This prospective multicenter randomized controlled trial included 64 children with partial-thickness second-degree scald or flame burns. Children were randomized equally to nano-silver dressing (n=32) or PRP injection (n=32). The main outcome of interest was the extent of wound healing, as assessed by the Wollina Wound Score. The secondary endpoints evaluated were pain severity, hospital length of stay, occurrence of complications, patient satisfaction, and scar-related results. Results: The groups were comparable at baseline with respect to demographic variables, burn-related characteristics, and laboratory parameters. Hospital stay was significantly shorter with nano-silver dressing (16.19±3.12 vs. 19.97±3.41 days; p<.001). Pain scores were significantly lower at 1 week (3.44±0.50 vs. 5.69±1.00) and 1 month (2.28±0.46 vs. 4.41±0.95) (both p<.001), and the Wollina Wound Score was significantly higher (5.34±0.97 vs. 4.47±1.22; p=.002). Satisfaction differed significantly between groups (Monte Carlo exact p=.017). Vancouver Scar Scale scores were similar at 3 months (10.7±1.69 vs. 10.8±1.67; p=.813) but were significantly lower with nano-silver dressing at 6 months (2.4±1.22 vs. 6.8±1.71; p<.001). Early and late complication rates did not differ significantly. Conclusion: In this pediatric cohort, nano-silver dressing was associated with shorter hospital stay, lower pain scores, more favorable wound-healing scores, greater satisfaction, and a lower 6-month VSS compared with PRP injection. These findings should be interpreted cautiously given the relatively small sample size and methodological limitations and require confirmation in larger, prospectively registered multicenter trials.",
