"abstract": "Background\n\nTertiary-care data can provide critical insights into clinical demographics in Gulf Arab settings where these data remain limited, despite reflecting hospital-based frequencies rather than population-level prevalence. This study aimed to descriptively characterize cardiometabolic disease and medication frequencies in a tertiary-care cohort.Methods\n\nThis retrospective cross-sectional chart review included adult patients with ≥1 ICD-10-coded chronic cardiometabolic condition at Mubarak Hospital, Kuwait, between January 1, 2022, and December 31, 2024. Disease and medication frequencies were summarized by age group (<35 years, 35 to < 65 years, and ≥65 years) and sex. Univariate and multivariate associations were evaluated. Binomial regression models estimated adjusted prevalence ratios (aPRs) and 95% confidence intervals (CIs), adjusting for age or sex, nationality, and smoking status, with further adjustment for ischemic heart disease (IHD) in selected models.Results\n\nAmong 501 patients (54.1% female; mean age 63.7 ± 14.6 years), the most frequent conditions were hypertension (77.8%), diabetes mellitus (58.5%), dyslipidemia (44.7%), and IHD (23.6%). Lipid-lowering medications (76.05%) and angiotensin-converting enzyme inhibitors/angiotensin receptor blockers (59.5%) were the most common medications. Disease frequencies increased significantly with age. Compared with those aged 35 to <65 years, patients aged ≥65 years had higher adjusted prevalences of type 2 diabetes (aPR: 1.17, 95% CI: 1.00-1.35), hypertension (aPR: 1.18, 95% CI: 1.07-1.30), and IHD (aPR: 2.37, 95% CI: 1.68-3.35). Males had a significantly higher adjusted prevalence of chronic kidney disease (aPR: 1.96, 95% CI: 1.20-3.19) and IHD (aPR: 1.69, 95% CI: 1.22-2.32) than females. Notably, the higher prevalence of aspirin/clopidogrel antiplatelet therapy in males (aPR: 1.37, 95% CI: 1.11-1.69) persisted even after further adjustment for IHD (aPR: 1.30, 95% CI: 1.05-1.60).Conclusion\n\nWithin this selected cohort, the cardiometabolic burden was high and varied by age and sex. Multicenter Gulf studies with patient-level adjustment are needed to characterize the disease burden and prescribing frequencies.",
