  "abstract": "Background\n\nFrailty is a critical geriatric syndrome reflecting health vulnerability, yet research among residents with chronic schizophrenia remains limited. This study investigated the prevalence and associated factors of frailty in a psychiatric nursing home.Methods\n\nA cross-sectional survey was conducted with 155 participants from a psychiatric nursing home. Assessment tools included the Frailty Scale, BSRS-5, PANSS, and Barthel Index (ADL/IADL). Data were analyzed using t-tests, Pearson correlation, and multivariable logistic regression.Results\n\nThe prevalence of frailty was 55.5% (86/155). Multivariable logistic regression revealed that female gender (Model 1: adjusted OR = 4.39, 95% CI = 1.92-10.04; Model 2: adjusted OR = 4.33, 95% CI = 1.90-9.85; both p < 0.001) and higher psychological distress (BSRS scores; Model 1: adjusted OR = 1.94, 95% CI = 1.35-2.79; Model 2: adjusted OR = 2.01, 95% CI = 1.42-2.85; both p < 0.001) were significant independent associated with of increased frailty odds. Although the frail group exhibited significantly more severe psychiatric symptoms across all PANSS subscales (all p < 0.01), no significant differences were found in ADL or IADL scores between the two groups.Conclusions\n\nThese findings suggest that frailty represents a multidimensional vulnerability that co-occurs with psychiatric symptoms alongside preserved functional independence. Routine integration of frailty screening into psychiatric assessments is recommended to inform interdisciplinary care strategies.Clinical trial number\n\nNot applicable.",
