"abstract": "Aim\n\nWe performed this study to determine the cardiovascular-cause mortality rate and the predictive value of the plasma osteoprotegerin (OPG) concentration for cardiovascular-cause mortality in patients with end-stage kidney disease (ESKD) undergoing maintenance hemodialysis (MHD) during the first 2 years.Methods\n\nWe performed a prospective observational cohort study of 218 ESKD patients undergoing maintenance hemodialysis from September 2021 to September 2025. Plasma OPG concentrations were quantified using a colorimetric measurement method based on ELISA method. Cardiovascular-cause mortality data were collected over 2 years of follow-up. All patients were divided into 2 groups: the survival group (n = 187) and the mortality group (n = 31).Results\n\nThe incidence of cardiovascular-cause mortality was 14.2% over 2 years of follow-up. Compared with survivors, patients who died from cardiovascular causes had significantly lower HDL-C levels and higher PTH, hs-CRP, and OPG levels (p < 0.001). In multivariable Cox regression analysis, higher OPG levels were independently associated with increased cardiovascular mortality (HR = 1.050, p < 0.001). OPG showed good discriminatory performance for cardiovascular mortality (AUC = 0.828, p < 0.001), and Kaplan-Meier analysis demonstrated significantly higher mortality among patients with OPG levels > 47.95 pmol/L (log-rank p < 0.001).Conclusion\n\nThe cardiovascular-cause mortality rate during the first 2 years of maintenance hemodialysis was 14.2%. Plasma OPG levels are associated with cardiovascular mortality, suggesting its potential as a useful biomarker for risk stratification in ESKD patients.",
