"abstract": "Purpose\n\nThe 12-month intraocular pressure (IOP)-lowering efficacy and tolerability of netarsudil 0.02%, a Rho-kinase inhibitor, were evaluated in Korean patients with primary open-angle glaucoma (POAG), including normal-tension glaucoma (NTG).Methods\n\nEighty-six patients (131 eyes) starting netarsudil at one center in November 2024-February 2025 were reviewed; entry required 3 months of follow-up, continued to 12 months. IOP was measured by Goldmann applanation tonometry at baseline and 1, 3, 6, 12 months (131, 131, 131, 107, 91 eyes). Efficacy was analyzed by diagnosis and treatment context: add-on, netarsudil added to an unchanged regimen, or switch, one agent replaced by netarsudil at that visit without washout. A linear mixed-effects model with a per-patient random intercept was used; eyes without a 12-month measurement were re-examined for discontinuation.Results\n\nFrom baseline IOP 17.34 ± 4.06 mmHg, the adjusted 12-month change was -3.05 mmHg (95% CI, -3.80 to -2.30), significant at every timepoint in POAG (-3.73) and NTG (-1.80 mmHg; all P < 0.001); the relative reduction was greater in POAG (19.0% vs 11.2%; P = 0.037). Netarsudil was discontinued within 12 months in 44 eyes (33.6%): 40 (30.5%) for an ocular adverse event, one for inadequate pressure response; Kaplan-Meier persistence was 86.3%, 69.5%, and 66.4% at 3, 6, and 12 months. Corneal verticillata was most frequent (54 eyes, 41.2%), asymptomatic. Among 91 eyes at 12 months, IOP fell ≥ 20% in 42.9% and reached ≤ 18 mmHg in 87.9%.Conclusion\n\nIn eyes already receiving ocular hypotensive therapy (mean baseline IOP 17.3 mmHg, over a third with NTG), adding or substituting netarsudil lowered IOP by a further 3 mmHg, sustained to 12 months in eyes continuing it. Discontinuation reflected tolerability, not loss of effect: a third stopped within a year, almost always from ocular surface symptoms - close to the registration programme but above other real-world reports.",
