"abstract": "Background: Mantle cell lymphoma is a rare B-cell lymphoma subtype with highly variable presentation depending on the patient. Recent studies such as TRIANGLE have brought important advances to the treatment of mantle cell lymphoma, mainly through the addition of BTK inhibitors and the growing question of the role of autologous stem cell transplantation in this setting. However, this access is not available to everyone, and many patients within the Brazilian Public Health System (Sistema Único de Saúde, SUS) do not have the opportunity for access, frequently requiring judicial court orders to obtain treatment. Aim: This study aims to evaluate patients with mantle cell lymphoma treated at Hospital Santa Izabel (an institution serving both public health system and private health insurance patients) to characterize this cohort and compare it with the literature and with recent real-world studies from other reference centers. Methods: This is a retrospective study evaluating medical record data from patients diagnosed with mantle cell lymphoma between 2019 and 2026 at our institution. This research is an analysis derived from a broader institutional hematologic patient registry approved by the Research Ethics Committee. Results: The analyzed cohort comprised 20 patients with the following demographic characteristics: median age 63.5 years (36–93), male predominance (60%), and most patients followed within the public health system (SUS, 75%). The median Ki-67 in the samples evaluated was 40% (10–95%), with a proportion of patients with Ki-67 ≥30% of 75%. In this cohort, no patient was classified as low risk by the MIPI calculated at diagnosis, with 85% classified as high risk and 15% as intermediate risk. Ann Arbor stage IV at diagnosis was observed in 80% of the cohort. Regarding first-line treatment, most patients did not have access to anti-CD20 therapy (rituximab) — 60%. Overall survival at 24 and 36 months was 74% and 68%, respectively, with the median not reached. Progression-free survival in the evaluated cohort (19 patients) was 20 months. Among patients of eligible age for autologous stem cell transplantation (≤70 years, 14 patients), a minority (21.4%) underwent the procedure. Conclusion: This cohort presents a higher-risk profile that, despite all the access barriers faced within the public healthcare system, still achieves outcomes comparable to the literature — offering hope that, with greater investment in public health policy, better responses may be achieved through universal access to more advanced therapies and technologies within SUS.",
