"abstract": "Introduction: Double gallbladder is an unusual congenital anomaly, exceptionally rare when located on the left-lobe 4b segment of the liver, left to the falciform ligament and draining via a common cystic duct into the common bile duct, with one gallbladder complicated by mucocele. Laparoscopic cholecystectomy is the treatment for symptomatic cases, and a missed diagnosis can lead to complications like biliary injuries.\n\nCase presentation: We report a case of ectopic double gallbladders in a 60-year-old female who presented with intermittent epigastric pain radiating to the shoulders, accompanied by nausea and worsening after heavy meals for the last 4 months. Ultrasonography imaging revealed a double gallbladder with cholelithiasis. The patient underwent laparoscopic cholecystectomy. Intraoperatively, two distinct gallbladders with cholelithiasis were found in the left lobe of the liver, with separate cystic ducts merging into a common cystic duct. Laparoscopic cholecystectomy was done uneventfully.\n\nConclusion: This case emphasizes the rarity of ectopic gallbladder duplication and the need for meticulous preoperative imaging and intraoperative caution. Awareness of such anomalies is crucial to minimizing surgical complications and ensuring optimal outcomes.",
