"abstract": "Objective. To investigate the characteristics of pathohistological changes in the wall of the proximal small intestine as the morphological basis for the development of intestinal failure syndrome in patients with acute pancreatitis.\n\nMaterials and methods. A pathohistological examination of specimens – sections and biopsies of the small bowel wall – was carried out in 53 patients with acute pancreatitis, of whom 30 (57 per cent) had a moderately severe course and 23 (43 per cent) had a severe course. Specimens were obtained during fibrogastroduodenoscopy in 17 (32%) patients, during surgery in 24 (45%) and at autopsy in 12 (23%).\n\nResults. Abnormalities in the structural elements of the small bowel wall manifested as signs of varying degrees of severity, extent and depth of inflammation: ranging from chronic – in 9 (17%) patients – to acute infiltrative–oedematous – in 7 (13%) – and diffuse enteritis with a component of destruction and erosive changes – in 37 (70%). Values of the villus height-to-crypt depth ratio ranging from 2.4 to 2.2 corresponded to a moderately severe degree of intestinal failure syndrome, whilst values ranging from 2.1 to 1.4 corresponded to a severe degree, which may serve as an objective criterion for assessing its severity.\n\nConclusions. Intestinal failure syndrome is one of the organ-specific complications of acute pancreatitis. The identified changes in the microstructure of the small bowel wall constitute its morphological basis.",
