"abstract": "Transient troponin elevation is a common observation in various nonischemic cardiovascular diseases, the origin of which remains widely unknown. We hypothesized that troponins enclosed in extracellular vesicles (EVs) and released by cardiomyocytes into circulation account for distinct disease-specific compartmental troponin signatures. In vitro, hypoxic cardiomyocytes secreted EVs enriched with cardiac troponin T (cTnT). Mice after myocardial infarction showed increased blood EV concentration, and pigs under induced atrial fibrillation showed distinct EV-cTnT signatures. These specific EV-cTnT and cardiac troponin I compartmental patterns were verified in 2 patient cohorts of myocardial infarction and atrial fibrillation, introducing relative EV-cardiac troponin as a novel biomarker to discriminate cardiovascular pathologies.",
