"abstract": "ObjectiveTo investigate the correlation between cardiorespiratory fitness （CRF） and lower limb muscle strength in patients undergoing maintenance hemodialysis， and to identify the independent factors affecting peak oxygen uptake.MethodsA single-center retrospective analysis was conducted. A total of 330 patients undergoing maintenance hemodialysis who were admitted to the Department of Nephrology of the First Affiliated Hospital of Xinjiang Medical University from January 2024 to January 2025 were selected as the disease group， and 326 matched healthy individuals from the same period were included as the healthy group. CRF was assessed by cardiopulmonary exercise testing， lower limb muscle strength was quantified by knee extension strength measurement， and exercise endurance was reflected by the 6-minute walk distance test. Differences in each indicator were compared between the two groups， and multivariate linear regression analysis was used to further explore the key factors affecting peak oxygen uptake in patients undergoing maintenance hemodialysis.ResultsCompared with the healthy group， patients in the disease group showed significantly poorer performance in key physical fitness indicators： peak oxygen uptake （［14.33±2.68］ mL·kg⁻¹·min⁻¹ vs ［21.36±2.76］ mL·kg⁻¹·min⁻¹）， knee extension strength （［26.65±3.69］ kg vs ［43.49±5.65］ kg）， and 6-minute walk distance （［419.65±37.98］ m vs ［533.32±24.56］ m） were all lower， whereas the timed up and go test （TUGT） time （［12.39±2.98］ s vs ［8.95±2.45］ s） was longer （all P<0.05）. Correlation analysis showed that peak oxygen uptake was positively correlated with knee extension strength and 6-minute walk distance （r=0.631， 0.589； P<0.05）， and negatively correlated with TUGT （r=-0.613， P<0.05）. Multivariate linear regression further confirmed that age， reduced muscle mass， and decreased muscle strength were independent risk factors affecting peak oxygen uptake （P<0.05）， and there was no significant collinearity among variables （all VIF<5）.ConclusionCRF is closely associated with lower limb muscle strength in patients undergoing maintenance hemodialysis. Advancing age， reduced muscle mass， and decreased muscle strength are key factors affecting peak oxygen uptake. Targeted intervention to improve lower limb muscle strength can effectively increase peak oxygen uptake， thereby improving exercise endurance and quality of life， and may provide important guidance for optimizing clinical rehabilitation strategies.",
