"abstract": "The adoption of Remote Patient Monitoring (RPM) is reshaping healthcare delivery by enabling continuous tracking of patient health data outside conventional clinical settings. RPM facilitates proactive management of chronic diseases, supports complex care needs, and improves patients’ daily lives by helping them manage their own health. By utilizing connected devices, RPM enhances patient autonomy, reduces barriers to care access, and mitigates risks associated with in-person visits — a benefit significantly underscored during the COVID-19 pandemic, which accelerated the acknowledgment of telehealth’s added value among health system stakeholders. These tools contribute to greater prevention, lower hospitalization rates, and fewer interruptions in patients’ follow-up, providing practitioners with a more constant stream of data for a clearer picture of their patients’ health. Despite its transformative potential, integrating RPM into routine healthcare workflows poses significant challenges, including its alignment with existing care models, patient adoption and engagement, the adaptation of clinical teams, and the sustainability of financial and operational frameworks. Particular challenges remain in making RPM accessible to vulnerable populations. Conducted as part of the Harkness Fellowship in Healthcare Policy and Practice at the HiBRID Lab (NYU Langone Health) and the Bipartisan Policy Center, this study examines the deployment of RPM within NYU Langone Health between August 2023 and August 2024. Drawing on stakeholder interviews, field observations, and document analysis, it investigates key factors shaping RPM’s effectiveness, scalability, and long-term integration, with the aim of informing public health policies to improve care quality and outcomes for vulnerable populations across countries.",
