"abstract": "Introduction: Recurrent Pregnancy Loss (RPL) is a reproductive condition with a multifactorial etiology that may be associated with hematological, immunological, thrombo-inflammatory, and hemostatic alterations. The evaluation of hematological parameters may contribute to a better understanding of the mechanisms involved in the pathophysiology of RPL (ASRM, 2026). Objective: To characterize the hematological profile and levels of natural anticoagulant proteins in women diagnosed with RPL. Methods: A cross-sectional study was conducted among 287 women with RPL receiving care at specialized services in Salvador, Bahia, Brazil. Laboratory data, including complete blood counts and natural anticoagulant proteins, were collected. Continuous variables were presented as mean ± standard deviation or median and interquartile range (IQR), according to the results of normality testing. Ethics Approval: CAAE 83666724.3.0000.8035. Results: Hematological parameters were within the reference ranges, with hemoglobin at 13.1 g/dL (IQR: 12.5–13.8), hematocrit at 38.3% (IQR: 36.2–40.2), total leukocyte count at 7.39 ×10 3 /µL (IQR: 5.63–9.13), and platelet count at 288 ×10 3 /µL (IQR: 237–330). Natural anticoagulant levels were preserved, with protein C (PC) at 104.8±20.8%, protein S (PS) at 87.3±19.0%, and antithrombin (AT) at 106.3±11.7%. Discussion: The apparent preservation of standard hematological parameters does not exclude more subtle alterations related to the inflammatory response and hematopoiesis. In a recent study of 531 women with RPL, those who experienced a subsequent pregnancy loss had higher erythrocyte, leukocyte, and lymphocyte counts, as well as alterations in RDW and MCV. Some of these parameters remained associated with the outcome after multivariate adjustment (MU et al., 2025). Together with the findings of the present study, these results suggest that blood count assessment in RPL may be more informative when considering not only absolute blood cell counts but also the patient’s inflammatory and metabolic context. Levels of PC, PS, and AT remained within the reference ranges. Similar findings were reported by Mekaj et al. (2015) in women with RPL, among whom no significant differences were observed in the mean levels of these anticoagulant proteins compared with controls. However, the combined occurrence of alterations in AT, PC, PS, and/or activated protein C resistance was more frequent among patients with RPL than among controls (21.15% vs. 5.45%). These findings suggest that an analysis based on mean values may not identify hemostatic alterations present in subgroups of women with RPL, highlighting the importance of individually assessing the components of the natural anticoagulant system. Conclusion: Women with RPL had hematological parameters within the reference ranges. The combined assessment of inflammatory and hemostatic markers may contribute to a better understanding of RPL.",
