"abstract": "This narrative review analyses the role of the ‘brain–gut’ axis in the pathogenesis of complications following abdominal surgical procedures. The biphasic neuroendocrine response to surgical stress, as described by Y. Naito and colleagues (1991), is examined, involving a transition to adrenocorticotropic hormone-independent hypercortisolaemia, which potentially inhibits tissue regeneration. The mechanisms of the vagus-mediated cholinergic anti-inflammatory reflex, the role of ‘surgical dysbiosis’ and the possible significance of butyrate deficiency in the destabilisation of the blood-brain barrier, as well as the pathogenesis of perioperative neurocognitive disorders, postoperative ileus and chronic visceral pain through the lens of neuroimmune interactions. The prospects for the use of Enhanced Recovery After Surgery (ERAS) protocols and psychobiotic preparation of patients as potential methods of targeted neuromodulation to optimise surgical outcomes are outlined.",
