Improving Hepatic Functional Reserve in Patients with Haemophilia and HIV/HCV-associated Cirrhosis: A Multicentre Prospective Study of Prolonged Foscenvivint Therapy
Abstract
over 3-4 hours, twice weekly for 24 weeks. The primary endpoint was the change in the albumin-bilirubin (ALBI) score from baseline to week 24. Secondary endpoints included the Child-Pugh score, liver stiffness (vibration-controlled transient elastography and magnetic resonance elastography), serum fibrosis markers, and safety with specific monitoring of bleeding events.Patients Five adult males with hemophilia A (mean age 59.2 years) and HIV/HCV-associated cirrhosis, all with a sustained virologic response ≥48 months before enrolment. Results Four patients completed the treatment; one discontinued treatment at week 22 due to non-treatment-related pyogenic arthritis. ALBI score improved in all evaluable patients, with mean changes from baseline of -0.420±0.130 at week 24 and -0.318±0.302 at week 28, four weeks after treatment completion. The serum albumin levels increased during treatment and remained above baseline at week 28. No bleeding- or treatment-related serious adverse events occurred. Conclusion Prolonged foscenvivint was well tolerated and it was associated with an improved hepatic functional reserve during treatment in this biopsy-infeasible population. These findings are hypothesis-generating and suggest that noninvasive functional endpoints may be a feasible evaluation strategy, which requires confirmation in controlled studies.
