Abstract

Background

Peroneal neuropathy at the fibular head (PNFH) is often associated with recent weight loss (WL); however, it remains unclear how weight-loss characteristics are related to nerve involvement. This study aimed to investigate the relationship between WL characteristics and clinical and electrodiagnostic findings in patients with PNFH.Methods

Thirty-seven adults diagnosed with PNFH between October 2018 and June 2022 were retrospectively analyzed. Patients were divided into WL (n = 22) and non-WL (n = 15) groups. WL was defined as a ≥ 5% reduction in baseline body weight within one to six months. WL parameters included absolute WL, WL percentage (WLP%), WL per month (WLPM), and BMI reduction (BMI-R). Clinical and electrophysiological findings were compared between groups. Results

The mean WL was 21.5 ± 13.2 kg over a median of 3.5 months. Overall electrophysiological findings were similar between groups, except for a higher prevalence of abnormal superficial peroneal nerve conduction studies in the WL group (10/22 vs. 2/15, p = 0.043). Sensory nerve conduction velocity showed significant negative correlations with admission weight (p = 0.005), pre-weight-loss BMI (p = 0.009), total weight loss (p = 0.007), WLP% (p = 0.008), BMI-R (p = 0.047), and WLPM (p = 0.046).Conclusions

Rapid or substantial WL is associated with more frequent sensory abnormalities in PNFH. Evaluating WL history and sensory nerve involvement may improve the clinical assessment of patients with PNFH.
