Abstract

Rachel Shemesh,1,2 Ofir Ohana,1 Omri Frige,1,2 Ruth Huna-Baron1– 31Department of Ophthalmology, Gray Faculty of Medical and Health Sciences, Tel Aviv University, Tel Aviv, Israel; 2Department of Arrow Project, Sheba Medical Center, Ramat Gan, Israel; 3Neuro-Ophthalmology Unit, The Goldschleger Eye Institute, Sheba Medical Center, Ramat Gan, IsraelCorrespondence: Ruth Huna-Baron, Department of Ophthalmology, Gray Faculty of Medical and Health Sciences, Tel Aviv University, Tel Aviv, Israel, Email [email protected]; [email protected]: Idiopathic intracranial hypertension (IIH) may result in optic nerve fiber loss and even atrophy. The precise timing at which optical coherence tomography (OCT) indices reach their lowest value (nadir), and the factors influencing nadir retinal nerve fiber layer (RNFL) thickness in the pediatric population, remain poorly defined. We aimed to determine the timing and OCT values at nadir in the pediatric IIH patients.Patients and Methods: We retrospectively reviewed medical records of pediatric IIH patients (age < 17 years) treated at a single medical center between 12/2009 and 1/2023. Data collected at baseline and at OCT nadir included lumbar puncture (LP) opening pressure (OP), body mass index (BMI), visual acuity (VA), visual field mean deviation (MD), OCT RNFL, ganglion cell complex (GCC) values, and clinical management. The worse eye data was analyzed. Results: Of the 26 patients, 20 (77%) were female. The cohort mean age was 12.6± 3.4 years, and mean BMI was 25.7± 6.5 kg/m2. Mean baseline RNFL thickness was 307.3± 125.3 μm. The mean time to nadir was 5.8± 2.9 months. Average RNFL and GCC thicknesses at the first appearance of nadir were 101.6± 16.3 μm and 81.7± 6.8 μm, respectively. A longer time to nadir tented to correlate with younger age (P=0.05). During a mean follow-up of 40.7± 33.7 months, 6 patients (23.0%) experienced a recurrence. Conclusion: Anatomical stabilization (nadir) in pediatric IIH occurs at approximately 5.8 months, which is earlier than reported in adults. In our retrospective small sample study from a single center there was some association between younger age and the value of nadir RNFL thickness. Further prospective studies with larger sample size are needed to validate our results. Keywords: idiopathic intracranial hypertension, optical coherence tomography, nadir retinal nerve fiber layer thickness values, nadir ganglion cell complex values, pediatric population
