Epidemiological study of hyperkalemia in patients undergoing hemodialysis and analysis of associated factors

Abstract

ObjectiveTo analyze the incidence, recurrence rate, and associated factors of hyperkalemia in patients receiving maintenance hemodialysis (MHD), and to provide an important reference for the prevention, treatment, and further research of hyperkalemia.MethodsData from MHD patients at Sichuan Academy of Medical Sciences & Sichuan Provincial People's Hospital were retrospectively collected from January 1, 2021 to October 9, 2024. Hyperkalemia was defined as serum potassium >5.0 mmol/L. The incidence and recurrence rate of hyperkalemia were calculated, and independent factors associated with hyperkalemia in MHD patients were analyzed.Results(1) A total of 433 patients with at least one serum potassium measurement were included, comprising 10 261 serum potassium results; the proportion of hyperkalemia was 31.3%. (2) The annual recurrence rates of hyperkalemia from January 1, 2021 to December 31, 2023 were 69.3%, 71.1%, and 72.5%, respectively. (3) Factor analysis showed that secondary renal disease, non-autologous arteriovenous fistula dialysis access, use of renin angiotensin-aldosterone system inhibitors (RAASi), and laboratory indicators such as high phosphorus, high magnesium, high chloride, low urea reduction ratio (URR), low triglycerides, and low sodium were important risk factors for hyperkalemia in MHD patients.ConclusionThe incidence and recurrence rate of hyperkalemia are high in MHD patients. In addition, the type of primary kidney disease, dialysis access, multiple electrolyte levels, URR, triglycerides, and RAASi use are all closely associated with the occurrence of hyperkalemia.
