Relationship between transforming growth factor-β1, microRNA-29c in peritoneal dialysate and peritoneal fibrosis in patients with glomerular diseases receiving peritoneal dialysis

Abstract

ObjectiveTo explore the relationship between transforming growth factor-β1 (TGF-β1), microRNA (miR) 29c, and peritoneal fibrosis (PF) in peritoneal dialysate of patients with glomerular diseases receiving peritoneal dialysis.MethodsA total of 184 patients with glomerular diseases were prospectively selected from Second Hospital of Shanxi Medical University from January 2021 to March 2023. All patients progressed to end-stage renal disease and received peritoneal dialysis, with a 2-year follow-up period; 4 patients were lost to follow-up. Based on whether PF occurred, patients were divided into a fibrosis group (n=48) and a non-fibrosis group (n=132). Clinical data and the expression levels of TGF-β1 and miR-29c in peritoneal dialysate were compared between the two groups before dialysis and 3 months after dialysis. Logistic regression analysis was used to identify factors associated with PF occurrence. Smooth curve fitting was used to analyze the relationship between TGF-β1 and miR-29c levels in peritoneal dialysate and PF occurrence. Receiver operating characteristic (ROC) curve analysis and the area under the curve (AUC) were used to evaluate the predictive value of TGF-β1 and miR-29c in peritoneal dialysate for PF occurrence.ResultsCompared with the non-fibrosis group, the fibrosis group had a higher incidence of peritonitis ([0.37±0.12] times/year vs [0.26±0.10] times/year), a higher dialysate-to-plasma creatinine ratio at 4 h (D/PCr) (0.72±0.17 vs 0.57±0.13), and a higher dialysate-to-dialysate glucose ratio at 0 h (D/D0) (0.53±0.14 vs 0.41±0.11) (P<0.05). After 3 months of dialysis, the fibrosis group showed higher TGF-β1 levels in peritoneal dialysate ([382.67±67.28] ng/L vs [325.94±62.31] ng/L) and lower miR-29c levels (0.58±0.18 vs 0.81±0.23) than the non-fibrosis group (P<0.05). After adjusting for peritonitis incidence frequency, D/PCr at 4 h, and D/D0, TGF-β1 and miR-29c levels in peritoneal dialysate 3 months after dialysis remained independent factors associated with PF occurrence (P<0.05). After 3 months of dialysis, TGF-β1 levels in peritoneal dialysate had a positive linear relationship with PF occurrence, and miR-29c levels had a negative linear relationship with PF occurrence (P<0.05). After 3 months of dialysis, the AUCs for predicting PF with TGF-β1 alone, miR-29c alone, and their combined use were 0.776, 0.732, and 0.884, respectively.ConclusionIn patients with glomerular diseases receiving peritoneal dialysis, TGF-β1 and miR-29c levels in peritoneal dialysate show linear relationships with PF occurrence. The combined use of the two indicators helps improve the predictive value for PF occurrence.
