Abstract

Background: Mechanical ventilation is a cornerstone of critical care, and one third ofintensive care unit. patients require it via an endotracheal tube. Accidental extubation is anunintentional dislodgement of the endotracheal tube that can cause hypoxia, airway trauma, emergency reintubation, prolonged ventilation, and increased mortality and cost. Aim: Toevaluate the effect of endotracheal tube care strategies on accidental extubation amongmechanically ventilated patients. Design& Setting: A quasi-experimental design wasutilized in the Psychiatric Neurology and Neurosurgery center, Tanta University. Subject:A purposive sample of 80 adult mechanically ventilated patients was equally divided intoa control group, receiving routine ETT care, and a study group, receiving additionalstructured ETT care strategies and meeting all inclusion criteria implemented by theresearcher Tools: three tools were used for data collection. Tool I: Mechanically ventilatedpatient assessment tool, ToolII: Unplanned extubation risk assessment tool. Tool III:Endotracheal tube care outcome assessment tool. Results: Accidental extubation occurredin 37.5% of the study group compared with 57.5% % of the control group. The study groupshowed improvement in consciousness level, more favourable sedation profiles, lowerextubation-risk scores, better airway clearance, and stable hemodynamic compared withcontrol group. Conclusion: Structured endotracheal tube care strategies effectively reducedaccidental extubation rate, maintaining hemodynamic parameters within normal range andMaintaining patent airway. Supporting the study hypotheses Recommendations:Endotracheal tube care strategies should be standardized within routine ICU nursingprotocols, supported by staff training and vigilant securement practices.
