Abstract
Objective of the review to systematize of scientific and clinical data regarding the etiology, pathogenesis, clinical manifestations, specificity of diagnostics, and therapeutic correction strategies of iron metabolism disorders. Materials and methods. The methodology is based on a systematic analysis of medical literature. The search for primary sources was conducted in the electronic scientometric databases PubMed, Scopus, Web of Science, and the Cochrane Library. The chronological framework of the analysis covers publications for the period from 2021 to 2026. Original clinical trials, systematic reviews, and relevant clinical guidelines of international medical organizations were included in the sample. Analysis of the literature. Iron deficiency anemia and preclinical forms of trace element deficiency are multifactorial pathologies. Abnormal uterine bleeding remains the main etiologic factor of trace element loss in women of reproductive age; however, the pathogenetic contribution of non-gynecological factors is confirmed. These include nutritional disorders, Helicobacter pylori infection, inflammatory pathologies of the gastrointestinal tract, absorption disorders, obesity, and iatrogenic effects. Special attention is given to the molecular mechanisms of regulation, where the hepcidin-ferroportin axis controls systemic iron homeostasis against the background of inflammation. The nonspecific clinical picture of iron deficiency mimics other nosologies. Differential diagnosis depends on laboratory parameters and concomitant symptoms. Conclusions. The clinical management of patients with iron deficiency and iron deficiency anemia requires an integrated approach. First-line therapy involves the prescription of oral iron preparations, the dosage and regimen of which must comply with current standards to minimize gastrointestinal side effects. Intravenous forms are indicated in cases of confirmed absorption impairment, intolerance to oral forms, or clinical necessity for rapid replenishment of tissue stores. An interdisciplinary approach to diagnosis and treatment ensures accurate identification of the disease’s root cause, which minimizes the risk of clinical relapses.
