Abstract

Objective To compare the differences in ultrasound imaging features based on the Chinese Thyroid Imaging Reporting and Data System(C-TIRADS)between mummified thyroid nodules(MTN)and papillary thyroid carcinoma(PTC),in order to improve the differential diagnostic capability of ultrasound for these two entities. Methods Patients who underwent thyroid ultrasound examination at Tongzhou Hospital Affiliated to Nantong University from January 2025 to February 2026 were enrolled as study subjects. Using pathological diagnosis as the gold standard, patients were divided into a MTN group(47 cases)and a PTC group(61 cases). The C-TIRADS ultrasound imaging parameters of the nodules were compared between the two groups, including maximum diameter, location, internal echogenicity, shape, margin, calcification type, calcification location, presence of the internal black-and-white double-layer halo sign, and internal blood flow signals. Binary logistic regression analysis was used to identify independent risk factors for MTN. Results There were statistically significant differences between the MTN group and PTC group in terms of nodule shape, calcification type, calcification location, internal black-and-white double-layer halo sign, and internal blood flow signals(P<0.05). No statistically significant differences were found in maximum diameter, location, internal echogenicity,or margin(P>0.05). Binary logistic regression analysis revealed that horizontal orientation(OR=2.358,95%CI:1.080-5.149,P=0.031),peripheral calcification(OR=9.857,95%CI:3.076-31.585,P<0.01),and presence of the black-and-white double-layer halo sign(OR=8.703,95%CI:3.627-20.886,P<0.01)were positively correlated with MTN independently, whereas blood flow signal(OR=0.212,95%CI:0.094-0.481,P<0.01)and microcalcifications(OR=0.313,95%CI:0.126-0.776,P=0.012)or absence of calcification(OR=0.246,95%CI:0.085-0.707,P=0.009)were negatively correlated with MTN independently. Conclusion There are significant differences in C-TIRADS ultrasound imaging features between MTN and PTC in terms of nodule shape, calcification type, calcification location, the black-and-white double-layer halo sign, and internal blood flow signals. These differences are helpful for differential diagnosis and for guiding the development of appropriate clinical management strategies.
