Abstract
Objective To analyze the independent risk factors for acute kidney injury(AKI)in patients with traumatic rhabdomyolysis(TR)without shock, and to evaluate the predictive value of cystatin C(CysC)and the sarcopenia index(SI)for AKI in these patients. Methods A retrospective analysis was performed on the clinical data of 243 patients with non-shock TR admitted to the Department of Critical Care Medicine,Wuxi Ninth People's Hospital from January 2022 to July 2025. Patients were divided into AKI group(n=59)and non-AKI group(n=184)based on the occurrence of AKI. General clinical data, vital signs and laboratory parameters within 48 hours of admission were collected.Univariate analysis,LASSO regression, and multivariate logistic regression were used to identify the influencing factors for the occurrence of AKI,and the predictive performance of relevant indicators for AKI was evaluated via receiver operating characteristic(ROC)curve. Results Univariate analysis showed statistically significant differences between the two groups in terms of gender, diabetes history, creatine kinase(CK),creatine kinase-MB isoenzyme(CK-MB),serum creatinine(Scr),lactate dehydrogenase(LDH),myoglobin(Mb),cystatin C(CysC),β2-microglobulin(β2-MG),uric acid(UA),C-reactive protein(CRP),and aspartate aminotransferase(AST)(P<0.05). LASSO regression identified seven variables including gender,CK-MB,CysC,β2 - MG,CRP,AST,and SI. Multivariate logistic regression revealed that elevated CysC(OR=1.174,95%CI:1.085-1.270,P<0.01)and elevated SI(OR=1.125,95%CI:1.073-1.179,P<0.01)were independent risk factors for AKI. ROC curve analysis demonstrated that the areas under the curve(AUCs)of CysC,SI,and their combination for predicting AKI were 0.844,0.794 and 0.972 respectively, with the combined model exhibiting the better predictive performance. Conclusion Elevated CysC and SI are independent risk factors for AKI in non-shock TR patients. Their combined detection offers high predictive value for AKI,facilitating early identification of high-risk patients and timely intervention.
