Abstract
Objective To investigate the effect of the C-reactive protein-albumin-lymphocyte(CALLY)index on short-term prognosis in patients with acute ischemic stroke(AIS)after mechanical thrombectomy(MT),aiming to provide a simple and reliable tool for clinical prognostic assessment. Methods A retrospective analysis was performed on 137 AIS patients who underwent MT at Zhongda Hospital,Southeast University from March 2022 to March 2025.According to the modified Rankin Scale (mRS)score at 90 days after MT,patients were divided into a good prognosis group(mRS score 0-2,n=95)and a poor prognosis group(mRS score 3-6 ,n=42). Clinical data ,preoperative CALLY index ,and the National Institutes of Health Stroke Scale(NIHSS)score within 24 hours after MT were compared between the two groups. Multivariate logistic regression was used to identify risk factors for short-term prognosis after MT in AIS patients. Receiver operating characteristic(ROC)curves were constructed to evaluate the predictive value of NIHSS score and CALLY index for poor short-term prognosis after MT. Results Patients in the poor prognosis group were older, and had higher NIHSS scores, higher prevalence of hypertension, and higher levels of NT-terminal pro-B-type natriuretic peptide(NT-proBNP)and C-reactive protein(CRP),as well as lower serum albumin, lymphocyte count and CALLY index(P<0.05). Multivariate logistic regression analysis showed that elevated NIHSS score(β=0.411,OR=1.508,95%CI:1.247-1.824,P<0.01)and decreased CALLY index(β=-1.154,OR=0.315,95%CI:0.199-0.500,P<0.01)were independently associated with poor short-term prognosis after MT in AIS patients. ROC curve analysis demonstrated that the area under the curve(AUC)of NIHSS score and CALLY index for predicting poor short-term prognosis was 0.914 and 0.925,respectively. Conclusion The CALLY index is closely associated with short-term prognosis in AIS patients after MT,and can be served as a potential indicator for predicting poor short-term prognosis in this population.
