Abstract
•SLNB identified lymphatic drainage beyond standard elective volumes in nearly half of OTSCC patients.•SLNB may guide individualized postoperative radiotherapy, including omission of contralateral neck irradiation in selected patients.•Adapted plans significantly reduced estimated NTCPs for xerostomia and dysphagia.•SLNB may support individualized de-escalation and optimization of elective neck irradiation in OTSCC.
