Abstract
Objective To investigate the clinical characteristics of patients with stage ⅠB lung adenocarcinoma who underwent radical surgical resection and to analyze factors affecting their prognosis. Methods The clinical data of 105 patients who underwent radical surgery for ⅠB lung adenocarcinoma at Xuzhou Central Hospital from January 2016 to December 2020 were retrospectively analyzed. Patients were divided into an observation group(n=41)and an adjuvant therapy group (n=64)according to whether they received postoperative adjuvant therapy, and the clinical characteristics of the two groups were compared. According to the adjuvant treatment modality, the adjuvant therapy group was divided into a combination therapy subgroup(n=25),an adjuvant chemotherapy subgroup(n=27),and an adjuvant targeted therapy subgroup(n=12). The Kaplan-Meier method was used to plot survival curves, and the log-rank test was used to compare overall survival(OS)and disease-free survival(DFS)rates between the two groups and among the three subgroups, respectively. A Cox proportional hazards regression model was used to analyze factors affecting OS of stage ⅠB lung adenocarcinoma patients. Results Statistically significant differences were found between the observation group and the adjuvant therapy group in differentiation degree, number of dissected lymph nodes, intravascular tumor thrombi, and intralymphatic tumor thrombi(P<0.05). No statistically significant differences were found in the 5-year OS rate(73.2% vs 83.8%,P=0.193)or 3-year DFS rate(65.9% vs 70.3%,P=0.676)between the observation group and the adjuvant therapy group. No statistically significant differences were found in the 5-year OS rate or 3-year DFS rate among the three subgroups either(P>0.05). Multivariate Cox regression analysis showed that tumor location(middle and lower lobes)[RR:1.620(1.116- 2.352),P=0.011]and number of dissected lymph nodes >10 [RR:0.078(0.022- 0.279),P<0.01]were independent prognostic factors affecting patients 'OS.Conclusion Currently, the 5-year postoperative survival rate of patients with stage ⅠB lung adenocarcinoma remains unsatisfactory, and patients with high-risk factors for recurrence are more inclined to choose postoperative adjuvant therapy. Compared with the patients without adjuvant therapy, although the survival curves suggest that postoperative adjuvant therapy may prolong OS and DFS,the difference was not statistically significant. Tumor location and number of dissected lymph nodes are important factors affecting patients'prognosis.
