Abstract

Objective To evaluate the clinical efficacy of Zhengyuan capsules in improving appetite loss and cancer-related fatigue in patients with advanced lung cancer receiving standard chemotherapy. Methods Clinical data were collected from 110 advanced lung cancer patients who received chemotherapy in the Oncology Department of the First Affiliated Hospital with Nanjing Medical University from March 2025 to September 2025. Patients were divided into the control group(55 cases receiving standard chemotherapy)and the experimental group(55 cases receiving Zhengyuan capsules orally for 21 days in addition to the same standard chemotherapy of control group). Visual Analogue Scale for appetite/food intake and Brief Fatigue Inventory(BFI)were used to assess changes in appetite loss and fatigue severity on day 1(d1)and day 7(d7)of chemotherapy, while chronic inflammatory index fibrinogen-to-albumin ratio(FAR)was analyzed. Results Baseline characteristics showed no significant difference between the two groups(P>0.05). On d1 of chemotherapy, there was no statistically significant difference in appetite, food intake and fatigue score between the two groups(P>0.05). On d7 of chemotherapy, the experimental group demonstrated significantly higher appetite score[8(8,8)vs 2(2,2),Z=9.902,P<0.01]and food intake score[6(6,6)vs 3(3,3),Z=9.706,P<0.01],and significantly lower fatigue score[1(1,1)vs 4(4,4),Z=9.293,P<0.01]compared with the control group. The FAR in the experimental group was significantly lower than that in the control group[0.08(0.07,0.09)vs 0.10(0.08,0.10),P=0.031]. Using FAR=0.09 as the cutoff value,110 patients were divided into a high-FAR group(FAR≥0.09,n=57)and a low-FAR group(FAR<0.09,n=53). The high-FAR group had a higher d7 BFI score compared to the low-FAR group[4(1,4)vs 1(1,4),P=0.029]. Conclusion Zhengyuan capsules can significantly improve appetite loss and cancer-related fatigue in advanced lung cancer patients undergoing standard chemotherapy, and its mechanism may be associated with reducing chronic inflammation.
