Abstract

Introduction: The development of medications for blood disorders, such as aplastic anemia (AA) and blood deficiency syndrome (BDS), faces significant challenges due to the toxicity and limited efficacy of conventional therapies, such as immunosuppressants. Recent pharmaceutical advances have focused on using multi-omics tools (metabolomics, transcriptomics, and proteomics) to elucidate the mechanisms of action of traditional botanical complexes. Key therapeutic approaches involve the ability to simultaneously modulate multiple biological pathways, such as oxidative stress, systemic inflammation, and metabolic homeostasis. Objective: To analyze recent advances in the production of medicines for blood disorders. Materials and Methods: An integrative review was conducted using the ScienceDirect advanced search platform with the following filters: original articles published in English in journals covering pharmacology, immunology, biochemistry, and chemical engineering. Results and Discussion: Panaxadiol saponins (PND), derived from Panax ginseng, promoted hematopoietic recovery in aplastic anemia models, restoring white blood cell, red blood cell, and platelet counts. The key mechanism involved suppressing the NOD-like receptor/NLRP3 inflammatory axis and improving intestinal barrier integrity. Studies using Rehmannia glutinosa combined with Amomum villosum (AR) demonstrated superiority over the raw herb in treating BDS, promoting significant improvements in hemoglobin (HGB) and hematocrit (HCT) levels, as well as reducing apoptosis in the bone marrow and spleen through the inhibition of PI3K/Akt/MAPK pathways. Chemical analysis identified 31 major compounds, ensuring the stability and precision of the production method. The use of serum pharmacochemistry to identify components absorbed in vivo has revealed that herbal pairs ‒ such as Flos Puerariae and Semen Hoveniae (FS) exert hepatoprotective and metabolic protective effects through the activation of the PI3K/AKT/mTOR pathway. Although this study model focuses on alcohol-induced liver injury, it is applicable to the development of medicines aimed at restoring lipid homeostasis and combating inflammation driven by cytokines such as IL-11. Conclusion: Recent advances indicate that integrating traditional processing methods with high-resolution analytical technologies enables the creation of novel therapeutic agents for cytopenias and inflammatory diseases. The production of these medicines is evolving toward a model in which multicomponent complexity is rigorously controlled, aiming not only to treat hematological symptoms but also to restore gut-bone marrow axis homeostasis and regulate the systemic immune response.
