Abstract
Sepsis is a life-threatening syndrome caused by infection ,characterized by a complex interplay between immune dysregulation and metabolic reprogramming,in which lipid metabolism plays a critical role. Recent studies have shown that patients sepsis commonly exhibit elevated triglyceride levels and reduced cholesterol levels, particularly high-density lipoprotein(HDL).These changes not only reflect disease severity, but are also related to the regulation of inflammation and endotoxin clearance. Based on these findings, therapeutic strategies targeting lipid metabolism have gained increasing attention, including statins, fibrates, proprotein convertase subtilisin/kexin type 9(PCSK9)inhibitors,cholesteryl ester transfer protein(CETP)inhibitors,as well as reconstituted HDL and apolipoprotein A Ⅰmimetic peptides. Although some of these agents have demonstrated anti-inflammatory effects and potential prognostic benefits in experimental studies, clinical evidence remains limited. Future research should focus on lipoprotein functionality, disease stage ,and patient heterogeneity ,and integrate multi-omics and precision medicine approaches to facilitate the clinical translation of lipid-targeted therapies in sepsis.
