Correlation of TyG and NLR with early neurological deterioration after thrombolysis in patients with acute ischemic stroke
Abstract
Objective To investigate the effect of triglyceride-glucose(TyG)index and neutrophil-to-lymphocyte ratio(NLR)on early neurological deterioration (END)in patients with acute ischemic stroke(AIS)after intravenous thrombolysis and their predictive value. Methods A total of 274 AIS patients who received intravenous thrombolytic therapy in Xinxiang Central Hospital from January 1,2021 to December 31,2023 were retrospectively included.According to whether END occurred within 72 hours after thrombolysis, they were divided into END group(n=32)and non-END group(n=242). The baseline data and laboratory indicators of patients were collected, and the factors related to END were screened by univariate analysis. The independent risk factors were determined by multivariate logistic regression analysis, and the receiver operating characteristic(ROC)curve was drawn to evaluate the efficacy of TyG index and NLR alone and in combination for predicting END. Results Univariate analysis showed that there were statistical differences in the history of atrial fibrillation, the history of smoking, baseline National Institutes of Health Stroke Scale(NIHSS)score, fasting blood glucose, neutrophil count,TyG index and NLR between the two groups(P<0.05). Multivariate logistic regression analysis indicated that the history of atrial fibrillation(OR=24.475,95% CI:1.349-443.996,P=0.031),the history of smoking(OR=0.378,95%CI:0.148-0.968,P=0.043),baseline NIHSS score(OR=1.074,95%CI:1.027-1.123,P=0.002),TyG index(OR=8.434,95%CI:2.575-27.629,P<0.01),and NLR(OR=2.200,95%CI:1.495-3.236,P<0.01)were independent influencing factors for END. ROC curve analysis showed that the area under the curve(AUC)for TyG index in predicting END was 0.644(sensitivity 0.719,specificity 0.533),and the AUC for NLR was 0.669(sensitivity 0.563,specificity 0.781). The AUC for the combined prediction of the two was 0.758(sensitivity 0.844,specificity 0.624),which was significantly superior to that of either single indicator(ZTyG-combined=2.158,P=0.031; ZNLR-combined=2.014,P=0.044). Conclusion Elevated TyG index and NLR are independently associated with an increased risk of END after intravenous thrombolysis in AIS patients. The combined assessment of TyG index and NLR can improve early clinical risk stratification for END.
