Abstract

A 59-year-old man presented with fever, erythema multiforme, and pancreatic swelling. Laboratory tests revealed severe peripheral hypereosinophilia without a history of relevant medication or evidence of infection. Bone marrow analysis revealed no blast proliferation or leukemia-associated fusion genes. Notably, Wilms' tumor gene (WT1) mRNA was markedly elevated at 26,000 copies/µg RNA. Idiopathic hypereosinophilic syndrome was diagnosed, and oral corticosteroid therapy was initiated, resulting in rapid and complete improvement. This case highlights a rare presentation of idiopathic hypereosinophilic syndrome with high WT1 mRNA, which helps in the differential diagnosis between secondary (reactive) and idiopathic hypereosinophilic syndrome (HES).
